Select Request Type:

  1. Request a new card for a person
  2. Transfer a card from one person to another
  3. Change the access that a card has / Reactivate card
  4. Deactivate a card

Note: If you have several card requests to make, consider downloading this form, filling it out, and emailing it to Building Access Services. It'll be easier for everyone.
