Q&A for Keys and Building Access Cards
- Contact the Department/Building Key Coordinator to submit your key request.
- Only the department designee, Key Coordinator, completes the on-line Key Request.
- The Key Request Form is located at www.fpm.iastate.edu/keys/request/
- The following information is required - Name and one of the following: ISU Number, SSN or NetID.
- Key Requests processed by 3:45 will be available at the General Services Key Desk
after noon on the following business day.
- The supervisor or department head are required to approve key requests.
- Any additional required signatures will be indicated on your FP&M Key Request Signature Sheet
and must be obtained prior to keys being issued.
- Keys may be picked up from FP&M Building Access Services Key Desk, Monday through
Friday between 7:30 AM and 4:00 PM in the General Services Building, Room 108.
- Keys may only be picked up by the person to whom they are issued or their designee.
- Key Requests will be held at Keys Services for 30 days after the initial request.
- The Building Access Services Key and Card Desk are located inside the main door
(west side, center door) of the General Services Building.
- Contact the Department/Building Card Coordinator to submit a request to add access to your
building access card.
- Building Access Cards are issued to new employees from the ISU Card office when they
receive their ISU Card.
- Key Requests will be held at Keys Services for 30 days after the initial request. After 30 days,
the request is cancelled and the key returned to Key Storage. Arrangements can be made from Key Services, 4-4211.
- A new request is required to obtain the requested key(s).
- Use the on-line transfer process on the Key Request Form.
- Obtain the necessary signature for the FP&M Key Transfer Sheet and submit to FP&M, BAS Key Services.
- You may return your keys to the Key Desk at FP&M’s Building Access Services, located in General Services Building.
- The Department Key Coordinator can run a report of the current keys assigned to you.
- Lost or Stolen should be immediately reported to your department key/card coordinator and the Key Desk, upon discovery.
- A $20.00 fee will be assessed for each lost or unreturned key.
- Keys or Building Access Cards should be turned in to FP&M.
- Duplication of university keys is strictly prohibited.
- Keys must be requested through your Department Key Coordinator and submitted using the Key Request Form.
- Keys are not to be loaned out and must remain in the possession of the Key Holder.
- Have your department key requestor go to the Key Request Form and request another key,
using the option of replacement for a bent/broken/non-working key.
- If the lock is broken and not related to a key problem, call the Work Information Center at 4-5100.
- FP&M Building Access Services can make recommendations for an appropriate department key box.
- Departments can request a Key Box by submitting a Service Request.
- Keys stored in the Department Key box will be assigned to a specific individual who is responsible for
keys used within the department and they will be held accountable for returning and transferring keys.
- Obsolete keys are to be returned. Keys are still issued to the Key Holder and will be
assessed $20 per key if not returned to FP&M’s Building Access Services Key Desk.
- Lock changes can be requested by Department Deans/Chairs or their designee by submitting
a Service Request.
- Emergency lock changes can be requested by calling the Work Information Center at 4-4211
and are subject to all charges, including overtime, if necessary.
- Contractors may request metal keys through their FP&M Project Manager,
provided no other access can be provided for the contractor to complete their work.
