Lately we have had front edge limestone pieces that are adhered on and a few are coming loose so the adhesive is failing. One piece was glued back on last week by Country Landscapes but in the process it cracked in a couple spots and they don't necessarily want to be fixing these moving forward. I was hoping we could get a couple of these pieces from Nelson Construction or somewhere else to match and that way we have a spare piece or two plus I would like to replace the one that is cracked now as it doesn't look great. In the spring I would like to take a look at all of our downspouts coming off the roof of the boardroom and ballroom on the west side of the building. When I was cleaning them out this fall I noticed some joints where the caulking is wearing out or in some places completely gone and this might be contributing to some leaking we've had inside the building when we get hard rain. At least one drain is missing some metal flashing to direct the water away from the building that I would like to repair/replace as well. **This can be on hold until Spring. Mike McCoy has walked through this project with me. Installing a sink for Mike Olsen's research in room 175 ASC1. Mike Olsen needs to move his lab from Sweeney to make room for the Bio Medical Engineering teaching lab. Once the sink is installed we will move his equipment. Timeline is critical for BME teaching lab to start on time. Email from Mike regarding sink: Hi Sandy, I took a look at what it would take to get a sink installed in Room 175. I think we're looking in the $13-14k range. I know that's pretty expensive for a sink, so here's how I got to that number After reviewing the drawings for that area, all of the valved piping that we saw is either chilled water or lab gas. The nearest domestic water is in room 171 near the overhead door and emergency shower. EH&S said to include a maximum of $1000 for radiation testing of the two walls we need to penetrate for sanitary and water piping. They think it will come in less than this. I have included a pump for the sink to pump the sanitary to either the corner of room 177 or the trap we found in room 173. Both options seemed to be less expensive than cutting and replacing the floor in 175 to tie into that sanitary line. I have included a point of use electric water heater for the sink since it's so far from any domestic hot water. I have included electrical for both the pump and the water heater. The question about the loading dock (0127), whether it is possible to remodel it, and what (how much) it would take to remodel it is something that we will talk to FP&M about. Ideally, if they could remodel it in a way that combines spaces 0116B, 0126, 0126A, and 0126B and makes them accessible through one single access door that would improve both moving around the area and ease collaboration across both physical areas. I am not sure whether that ideal configuration is possible, but we can certainly inquire about it. I've been talking to the LAS administration and we would like to get new signs for the Men's and Women's restrooms on the basement floor of Catt Hall. The signs are currently for Men and Women, but we'd like to have them changed to Gender Neutral signage for inclusivity, since they are each just a single stalltoilet with a lock already in place on the door. I'm not sure the exact process required for this, but I thought I'd try here first.

College of Design Reading Room (111) and miscellaneous first floor office space (158, 158A) In fall 2023, we had engaged the services of an architecture firm, DLR, to help us understand the potential scope of this project. This project scope is limited to interior remodeling on our first floor of the College of Design. In particular the scope includes the remodeling of critical spaces for our ongoing operations and course work. The biggest project is the remodeling the former "Reading Room" space (Parks Library is shutting down operations and moving stacks out of the space) into design studios/classrooms. The first phase of this project will not involve any major demolition, extensive interior partitions, or changes in lighting, electrical, HVAC, sprinkler, or plumbing. It will simply involve new furniture, finishes, and AV installations which we can accomplish working directly with FPM. Eventually, we may have more extensive remodeling, based on what we identify as our long term use needs for this space--again, with the assistance of an architectural firm. There are other smaller miscellaneous interior remodeling projects to support our ongoing recruitment efforts (e.g., convert existing mailroom to open space space/waiting space for visitors, relocating mailroom elsewhere). Minimal changes in interior partitions and doors are necessary but not HVAC, electrical, sprinkler, etc. This will not require architectural services outside of the scope that FPM can provide. Schedule Considerations The ideal schedule for our Reading Room project m! ay not be possible--it depends, in part, on the timing of when Library will have all items moved out of our Reading room space (Room 111). Ideally if they move out by May, we can begin spending some of these funds towards the immediate reconfiguring of the space for our first phase of work. Mark Wyant has identified an issue with only one egress path from the south office corridor (door openings CC204, CC252, CC304 and CC352) that may impede the egress of mobility impaired individuals. We would like to have this situation reviewed. There may be potential conflicts with door swing, office door locations and fire ratings etc. Whatever solution is identified it should be reviewed with Troy Carey of EHS and may also require approval with the State Fire Marshal. Troy should be able to assist with that determination.

Add a new IT “pod” that needs dedicated cooling. Data center staff has designed a new pod layout to support high end research equipment. The pod will need a dedicated cooling system to manage the heat load created by the equipment. We need to install ACRC301S in row coolers that will connect back to the CDU. The CDU will need to be connected into the chilled water loops that support the data center. The CDU will connect to the in row with PEX pipe. Equipment has not been purchased yet as we need to confirm with the chilled water techs that we can install this system as we have designed. We would like to reconfigure the desk area space to make it more user friendly and to comfortably accommodate two work stations. 141 Tub Removal: I’d like to request a quote for removing a tub/water tank from Forker 141 and replacing the tub with a floor. The tub/water tank is roughly 6 feet deep and sits roughly two feet above floor level. The eventual purpose for the room will be a “dark room” for light sensitive research activities.

This started with FPM Building Maintenance and transferred to FDC Projects per Justin Abbott. Original Work Order #252000 Window doesn't latch. Drywall is damaged from window being open when it snowed. Per Justin Abbott: Water is coming in through the walls. Outside needs to be graded away from the building. 10/17/2023 - FPM Shops will be handling the service request for outside grading work.

Please submit a service request to FPM to evaluate some interior concrete repairs at Hilton Coliseum referenced in the attached quote. (exterior repairs are being handled separately). We are working on redesigning this space into a low distraction testing room. We are looking for it to sit around 18 students of varying mobility ability. We have a testing space in our office (1200 J) that we would like to model the room after but need help in the planning and excecution of this project. Please feel free to call me for more details. We are looking at updating our reception space in the main SAS office and wanted an opinion about what options there were for the space. Ideally, we would like to reuse most of the front desk, remove the unused section by the big windows, and add privacy pieces to the desk to hide the cords. We would also like to move the desk to the other side of the room, but it is a wired-in desk. We think the entire process would include repainting one of the walls, adding a decal to the wall, taking apart and moving the reception desk and wiring it in a different spot, getting additional privacy desk pieces, and potentially replacing the carpet as it may be unusable under the desk (but we are not sure).

We would like to order 3 chairs as similar as we can to the one in the attached picture. The most important features are adjustable to counter height; armless so they can go under the desk and they need a foot rest. If you could send me the options and price, I will get back to you with the order.

We need to create a secure lab space in the open area of 0328A using divider panels (we assume). It needs a minimum square footage of 500 square feet and have a lockable access door. It is not necessary that the dividers go all the way to the ceiling. Our current goal is March 15 for completion but this may change.

I would like to order cubicle walls to divide the supply storage part of the teaching lab from the working part of the teaching lab. The cubicle walls do not need to be too high, just discourage students from wandering to the back of the room and just helping themselves.

The department is interested in installing sound dampening in the hallways 2230, 2200, 2250. Noise from the Atrium events is becoming disruptive to the productive work environment of the faculty with offices on the 2nd floor. We would be intersted in options that both help absorb the sound and look decorative and make the space less industrial. Obviously function is the priority.

Intention is to have a red background with white lettering. A word document can be provided that helps illustrate the placement and verbiage for each sign. Convert Conference Room space into Lactation room. It currently has a sink but separate work order in place to update counter / fixtures. We are looking to update the door access by changing the lock, add some chairs (need), Desk (have), possible refrigerator and other accommodations for the space as appropriate.

Look at options for different tables. Current tables are 4' x 10' - Qty. 15. Looking at possible narrower tables to increase seating capacity. Not determined at this time if seating would be changed or not.

We would like to have the Engineering Team review the recently installed PRV for code compliance and safety in the mechanical room and develop a detailed plan to correct any deficiencies found that could be either shared with a contractors or in-house staff. An engineering probable cost estimated would be greatly appreciated.

High Priority - This came from FPM Building Maintenance Kildee Hall - Mechanical Room 0026 - Center floor drain is plugged and needs cleaned out with drain cleaning machine/snake. Drain issue that needs to be turned into a project. We are unable to clear this drain and when we plunge it water comes up around electrical conduits in the vicinity. This leads us to believe that we have a deteriorated pipe under the concrete. This will require a plumbing contractor that has the capability to demo concrete, make the repairs, and replace the concrete. Ben Adams has extensive knowledge about this problem and claims that it has been turned in as a project before.

We would like to replace desks and chairs in this room. Desks: 54"x30" height adjustable (manual) with butcher block tops. Chairs: height adjustable to desks, no arms Review storage space in the current area. This will encompass the west side of the King Pavilion on the lower floor. A student submitted a report to our office about incorrect braille signs in Lagomarchino. She shared that the floor signs have inaccurate braille on them and included a photo of the first floor sign. I do not have further information or knowledge about whether there were concerns about other signs or where exactly this sign is located in Lagomarcino.

We have a new faculty member, Han Na Lee, starting in Psychology this fall and we would like estimates to update/renovate two rooms in Lagomarcino Hall. Han Na will be in town on May 6 and available any time that day to be present for looking at spaces. The rooms to be updated are Lago 2311 (Office space) and Lago 1465 (Lab space).

We have a small project in Larch Hall where we need to add an enclosed kitchenette in the corner of the main lounge. We would like to submit our drawings to your design team in order for you to review them to send to the State Fire Marshalls office for an exemption from full review. Flood Damage Repair

Please start a project request to have the the 7 tier contactors switch over to metasys to be able to control the lighting. Please refer to Mike McCoy as he is familiar with the situation.

Building grounding system. The past electrical manager was tasked with obtaining a quote from DDM for this work. When I contacted them they told me that there was never a request from FP&M for a quote. Moving forward we will need a written scope of work for the grounding improvements before we can get a price from a contractor. This is something that we would not do in house at this time so this can go to the design team. I was told that Brian Dangelser and Mike Olson have more knowledge on the subject.

Please start a new project request for Parks Library. We would like to go over lighting options for all 7 tiers. We would like Troy Bowers to be the electrical engineer on the project. We would like to meet with several vendors (3E and M&M) to go over options for replacement lighting in all of the tiers.

FPM, Brandon Bartleson worked on this in Mid July and patched the exterior wall but this is still leaking. 293958 was the original work request that has already been closed out. Please look into this farther. Below the two large skylights in the SE corner of 002, and in the SE corner of the study rooms 003 and 004 after a rain the carpet is always wet from the wall to about 2' out. The wall is not visibly wet or dripping when this happens. We have had issue before judging by the peeling paint and multiple caulking jobs to the glass. This seems to be a foundation issue that needs to be addressed. Area is close to storm drain that has issues holding water or being clogged. See Justin Nolting or Shane Baumler for more information.

Parks Library Rm M0092C Provide hydraulic calculation plate for west riser (1980s) design area at the bottom of the riser in room M0092C. This is a result of 2024 Fire Marshall Inspection

Parks Library, Rm M0092C Riser control valve labeling needs to be adjusted to reflect sprinkler system changes. Some of the building wet zones are now on the Tiers Riser, so "Tiers" and "Main Building" tables are no longer accurate. Labeling should be as clear an unambiguous possible.

Bruce Bonestroo and John Byerly are familiar with this request. Lied Rec Center has foundation settling issues an they have reoccurred. Rec Services needs to have this reviewed and determine a corrective course of action. This will involve structures engineers and possibly civil engineers for the study.

Improving signage for the Lloyd veterinary medical center 1. the signs along the right hand side of Riverside Drive-the first sign displaying where to turn for small animal hospital is located too close to the entry of the employee parking lot leading to a number of client complaints that the signs are not clear - is it possible to move this sign further up riverside drive closer to the turn to enter the main small animal entrance/client parking? 2. I would like to add a lighted sign to the canopy over the entrance to the small animal hospital. The lighted sign would be similar to the lighted red and white plus sign on S. 16th Street. It would be a square, lighted with the red Plus and white background to better identify for owners in an emergency situation where to go.

This request is part of a multi building move of 130 total people moving to various CHS locations in preparation for the LeBaron project.40+ offices and approx. 90 people are moving out of LeBaron. There are moves within MacKay, Palmer and Lago to accommodate for program adjacencies. Most remaining moves are either moving people in or out of MacKay. MacKay is the building with greatest move activity. He feels that the adjustable legs that are on my current desk could be removed & added to a straight desktop. If that won't work, I will need a whole need straight desk part that is adjustable. Bike Maintenance Station Replacement

Rm 4530 - Install cubicle walls for two staff desks located in central/open area. One may just need privacy screens. Rm 4530 - Central/open area will be used as graduate student work spaces. We want to explore alternative arrangement for the existing furniture, addition of desk privacy panels and rolling drawers/file cabinets. Rm 4558 - This is currently a space for multiple grad students. It needs to be converted into a staff office. Desk needs to be extension to match other staff L-shaped desks. Rm 4567E - Additional locked cabinets are needed to store documents. Cabinets may not be located in this area but need to be easily accessible for staff and students working at the front desk.

Make and install approximately 17 new or update some existing gray plastic signs with white lettering to improve way finding of Biotech Facilities on ground and first floor. Sign locations and wording is already determined. I need a cost estimate before proceeding.

1210M MBB - Remove existing carpet and replace with new carpet tile. Occupant will remove all exiting furniture before work starts. Existing base is straight and does not need to be replaced.

Flood Damage Repair

Office and Lab Basement Tunnel Access through Room 13 - Bottom of the Stairs Broken sewer pipe in the tunnel area that needs heavy clean-up and repair. Pipe is broken in (2) separate locations, one area sewage flows freely.

The Lab School in the Palmer Building has several facility related needs. 1) Exterior work to tree roots and sidewalks on the playground which have become tripping hazards. 2) Increased need for security through alarm sensor and key code access. 3) Light construction for a ramp in one of the classrooms. 4) Updating some plumbing fixtures and countertop/cabinetry in the kitchen area. 7/27/2023 - adding scope Remove the outside drinking fountains at the Lab School.

Child Development Laboratory School - Outdoor playground for infants and toddlers (northernmost playground). The treated wood bridge has become unsafe for children to use. The wood is buckling, and the floor is separating from the railings. It immediately needs to be blocked off so children cannot access it, and it needs to be replaced to match the bridge/decking in the middle playground.

Updating some plumbing fixtures and countertop/cabinetry in the kitchen area

Purple accent wall needs to be painted a neutral color or updated accent color to match Palmer color palette. White board needs to be removed. May be some patch and paint. Leigh Wiand should be the FP&M contact.

Parking Lots-Pavement Management Plan-Multi-Year

Vet Med Plaza pavers Replace about 13 pavers this spring. If we approve a contractor now, they could pour the pavers ahead of time and be ready to install in March-April prior to graduation. I would also see if we would allow them to put a lifting eye lug in the unit for easier install removal and grout or plug it in when completed.

Please put together a project scope and budget to locate shut off valves on cold and hot domestic lines so that the building can be isolated in sections/areas. Eventually will need detailed drawings indicating valve locations and sizes as well as a recommendation for order of install.

Sign shop to update the Monument sign for Veterinary Medicine building to Update the building being renamed. Chris Strawhacker has details.

Provide new lounge furniture and backpack shelves in VM 1001 area just outside of 1010 Anatomy lab. The space has to accommodate the existing AED, printer and access to both elevator and stairs.

This is a request for an extension of the current hoist system track from the ceiling in the loading dock (Rm. 1674). The extension would be from the current end point of the track in the dock, extending out an extra ~10ft. towards the garage door to allow for proper coverage of roll off containers during body disposal.

Install approximately 10-12 electrical outlets on the north and east walls of the room adjacent to the farrier shop. These outlets will support farrier forges and power tool equipment. One outlet will need to be 220 for the welder.

Renovation of this general university classroom to become an Active Learning Classroom. This would include moveable tables and chairs, supplemental writing surfaces around the perimeter of the room and well as supplemental monitors. We are aware that with two walls of window the supplemental writing surface and monitors may not be possible. We were informed that the gate in this room cannot be locked for fire safety. We propose removing the lock into 57A, placing a lock on the gate, and updating the exit sign to point someone who got locked in to use the planetarium back exit. This would prevent the possibility of being locked into that area.

I am interested in getting a cost range estimate for installing electric ridge vent motors in Plant Pathology Greenhouse rooms B124, B126, B127, and B131. These rooms all currently have chain pulley opening and closing mechanisms. I salvaged all of the roof vent motors and controllers from the Forestry Greenhouse project last summer so we will not need to purchase those, just get them installed. If it is possible to get an estimate and complete the project by the end of June it would be preferable but not 100% necessary. Thank you!

We recently removed a large printing press that left behind a trench in the floor, that was used for wiring. This trench is 11 feet long by 12" wide x 16" Deep. There is also another hole in the floor that contains a pipe/conduit that leads underground to trench, about 6 feet away and 8" in diameter. We'd like to get estimates for filling in or covering this trench and hole.

I'd like to order a replacement door for the manager's office entering into Room 119. The current door was modified from a project in 2020 and no longer serves the needs of the office. A new door is needed, along with updated hardware to match other doors in Room 117, and include a keyed access lock. If possible, the retractable smoke shield for sound protection should be re used. An on site visit may be best to ensure replacement door can be similar to those in 117.

After an on-site review of our rooms with Wesley Gee, I would like to request having three ADA compliant room signage lenses manufactured. The clear room signage lenses should display the room numbers of 0119, 0122, and 0122A. Please see attached FP&M blueprint with corresponding plat numbers and approximate location of each room sign. Grey 8"x8" frames are already on-site, no additional frames are necessary. This is just for the ADA lenses with raise lettering and accompanying Braille.

I was not sure if this would be considered a project? We need two new signs for each side of our Ross 212 Conference Room. The current signs are 14"L x 5"H. 218 Ross Hall is currently more of a storage room right now, but we do keep a lot of DVD's stored in there for our Film Studies course instructor, among other miscellaneous items such as a couple of old desks, wheeled carts, etc. We are working on relocating the DVD's, and we can either Surplus some of the rest of the items, or move them to our other storage room, 318. We are interested in converting 218 to a dining type area for people to use for lunch or breaks. We will not have any appliances or countertop electrical items, we mainly just want some tables and chairs to make it a nice space in which to eat or gather.

Duane Strah stopped by today in reference to Request number 341913 which was for new signs needed for Ross 212. We talked with him about making a another new sign for what is going to be our new Lunchroom, 218 Ross. It will be exactly like the sign that hangs from the ceiling directing people to our mailroom, but it will say "Lunchroom", with arrows on each side pointing to the room. The Fire Marshall came through our building and requested we install automatic door holders on 603 and 618. Tim Kelley in FP&M said we would need to connect these door holders to a system in the basement of Ross and gave us a rough estimate. We have a 65" monitor that we would like to have mounted on the 2nd floor in the hallway, south wall, immediately preceding the 212 Ross Conference Room. We will need you to purchase the mount for the monitor. There is an electrical outlet on that wall toward the ground that will need to be relocated so it sits behind the monitor. Lastly, we would like to know an estimate on either purchasing a pre-made "box" to surround the monitor on each side to protect it from possible damage (if someone should accidentally catch their bookbag on it while walking through the hall, or other similar scenario), or if Carpentry could possibly make such a box.

Equipment room M020 heating water pump #12 is leaking. Repair\Replace. Confirm Scott Nibe. This has not been completed per Don Gregory - it is running on a drop cord. It has been turned in to have the electric updated and hooked to system controls. This project was sent from FPM Building Maintenance - Scott Nibe did some pump installation work and the last he knew the project was on hold until an electrical and Metasys solution for controlling the pumps can be found. So far the trades involved include, Facilities Mechanic, Electrician, Controls, Insulation. Please turn into a small project.

Per the Safety Inspection Report rooms in Science II may need an exit sign emergency lighting include 502, 504, 514, 515, 517, 523, 526, 602, 608, 615, 619, 623, 637, and 706. Dr. Craig Abel could use a second opinion from somebody knowledgeable at ISU about this kind of thing. Then have the work completed. These are all USDA Labs.

I need two signs installed in the Science 2 Prairie, pending approval from the University Architect. They are 30"x24" signs that would stand about 2.5-3', similar in structure to the monarch butterfly sign that sits in front of the Advanced Machinery Systems Lab or the Sloss House. As the prairie is subject to regular burns, the signs should also be at least partially removable (even if it's just the faceplates that have the information on them.)

The ongoing problem with the ceiling AC unit needs to be elevated to a project. Year after year the occupants of this space have to deal with various leaks from an AC unit that was poorly installed and constantly leaks. I would like to get some estimates and options for student lockers to be put in the hallways outside of the teaching laboratories in Science 1. The students can not bring coats into the lab and the lockers we currently have are very small and do not accommodate backpacks or big coats. I would also like the lockers to possibly have a key code for users to use for locking items. I need to know an estimate of this project and if it is feasible for us to move forward, what the time frame would be, etc..

There is a crack in the first floor glass blowing Gaffer's Guild SICTR 1202 that allows water to leak through concrete the floor into the basement Media Production Suite SICTR 0143, 0142 and 0142A. If there is any mopping or liquid spilled on the floor in the glass blowing area near this crack it leaks down into the media production suite where there is a significant amount of electronic equipment. This is a thick concrete floor so we were surprised it went all the way through. Please address ASAP!

SICTR Needs to replace a large white bar height table in the 1st floor atrium with as close as we can get to the other two we still have. Originally one of these was in 3121A if that helps identify the model. The chairs also have HON tags on them. LAS has offered to replace on their budget out of Start Something Academy Funds fund SG2705672 given they accidently damaged the original beyond repair, however that is not an option on the accounting string below. We have had multiple complaints of excessive noise from the air ducts in both room 1133 and 1118 in SICTR. Both rooms can get noisy enough people cannot hear each other without yelling when the air is flowing. Neither room has a false ceiling and if a wrap or padding added to the ducts in the rooms would not mitigate the noise level, it will likely require a barrier be added between the occupied area of the room and the duct work above. Remove existing 4th floor security grille and replace existing security curtain with a manually operated system. The new system would keep existing structural track with minimal lock down posts (5) to be operated outside of grille. The work costs includes modifications to pocket with support of modified track.

There is another loose aluminum cover panel hanging off the bottom of the NW corner of SICTR a couple panels down from where we replaced the last one on Request ID# 253498 last year. Picture attached. Last time this had to be turned into a small project to get it re-attached, PR1226471 (SICTR-General-330146-Exterior Repairs) as it apparently fell outside of the building project's warranty. Can we get this looked at, turned into another project if necessary, and ensure that no other panels will begin to fall off as well? We don't want this to continue to happen and cost $2700-$3100 each individual time.

We are looking for soft-sided furniture for the Margaret Sloss Center front lounge space where the 'kid's corner' is located. This furniture would be appropriate for the kids that come into the space, but also is sufficient to support adults, during times when the space is busier with students and no children are present. Specifically, we are looking at the soft-sided furniture from the Steelcase B-Free lounge series, with 2 or 3 small cubes (in yellow) surrounding the red Alight round ottoman.

1. Rec Services needs the lighting control system in State Gym updated from what is a non-supported system by Watt Stopper. We want to keep the panels, conduit and existing wiring in the building as much as possible to keep cost down. We need to upgrade the existing Watt Stopper Relays, and controls to the 10 panels as well as upgrade the Watt Stopper software that runs the system. 2. Replace the light fixtures in gymnasium 1250 (State Gym Expansion) with new LED fixtures that have motion sensor, dimming capability, wireless and ability to turn on and off at the site (not by computer). This would be the same system that was installed in State Gym East courts a few yeas ago.

Rec Service wants to explore the cost and logistics of providing a building wide sound system to play background music. This would be for State Gym first and second floor open fitness space. It would not include the pool, gymnasium or individual rooms. Depending on the results we would look at the Lied Rec Center for the lobby area and 3rd floor weight/fitness space.

John Byerly is familiar with this request. At the State Gym expansion there is what we believe is a condensation/freezing issue where the pool ceiling/roof meets the dry part of the building. This results in a "leak" when the frozen condensation melts and it is a significant amount. Part of this may involve removing some of the wall panels, looking at insulation, roofing and air flow.

State Gym Admin Office 1180M has an elevation desktop. We would like the side desktop to elevate as well. This was done in Office 1180S recently and Leigh Wiand worked on that project.

Replace office desktop with an elevating work top. There are two sections that need to be replace and replaced with TWO elevating stations. A upper 4 door cabinet needs to be removed. An upper 2 door cabinet needs to be relocated with mounting brackets. Two existing tact boards are not large enough to cover the space where the upper cabinets were located. Additional tact boards will be needed. I prefer that someone assigned to this project come over and put eyes on it since there are multiple items that need to worked out.

Sprinkler System. Rooms: 1201 & 1209. Please get estimate for a dry control valve serving sprinkler heads in vestibules, room numbers: 1201 and 1209. Current sprinkler line is charged "wet "and is prone to freezing, vestibule heaters are undersized for amount of traffic. Need pricing for dry valve to mitigate frozen heads.

There is a bolt that fell off of a structural beam on the outside of the building on the East over hang that looks out to the parking lot. There is gap forming. We need someone to come check it out. I will attach pictures.

Fire Escape Repairs - Fire Escape Inspection Report in the LX folder and G Drive folder. Bricks appear to be coming loose below the third-floor landing where the fire escape attaches to the exterior of the building. Have the fire escape inspected by a registered design professional and place particular emphasis on this area. By section 1104.16.5.1 this should be repeated every 5 years.

Repair/replace leaking pump, safety committee item - This was sent from FPM Building Maintenance This requires asbestos, and the pipe is rotting.

Eyewash station in 0209 Sukup needs attention. It takes abnormally long to clear rust particles during the monthly flushing. It is getting worse overtime. At its current condition, it probably requires daily flushing to keep the water clear of contaminants. I believe we need to install a water filter for that specific station. That station situates at the bottom of a straight pipe that connect eyewash stations on the upper floors. Sediments settle to the bottom which is the inlet for that particular station. Frequent flushing is not a long term solution. We need FP M help to install the filter or discuss options to fix the problem.

Northwest corner bricks are missing and need repair. The limestone cap on the retaining wall needs to be replaced. Appears that the planter soil on the backside maybe causing water not to drain from the flashing under the cap, then freezing. There is also open joints. Address Steam Boiler room issues noted during Pressure Vessel Inspection. To obtain pressure vessel certification; gas piping, gas train, and burner head must be replaced on boiler due to rusting and condition. Boiler room must be cleaned and dry; the water and moisture issue must be resolved to prevent further issues with the boiler. See Scott Rogers for further information. Additional issues in room: Boiler stack needs insulation to prevent heat loss into room. Steam piping above door to basement is leaking onto platform on stairs, unknown black tar substance is leaking down wall below platform. Air flow needs to be improved to help with moisture and ventilation. Hatch to outside was not closed allowing water/moisture into room. Lighting in boiler room works intermittently. Lighting on stairs also works intermittently while walking down steps making it unsafe. Recommend a walk-thru to note condition and issues of area.

The air handler in VMRI 02 has a broken steam coil for heating. This repair will also likely require the removal of duct work and steam piping. Mike Moore and Eric Zierke have more information if needed. This was originally a shop's work order 323485. Attention: Eric Ohrt

Estimate to replace generator for VMRI 40. This unit is an old International model with no support for replacement parts. Also the size of the generator should be checked, this one could be undersized. NTE $5000

This request is for concrete floor and wall modifications to the LAR Building 40 and LAR Central animal facility cage washer rooms to install a new cage washer in each facility. We have accepted the purchase bid for the cage washers and ASAP we need to have a contractor assess the concrete work needed done to accommodate the installation of larger replacement cage washer units. I recommend that we contact Story Construction for this assessment, estimate, and work. One of the cage washer vendors, that did not receive our bid, had Story Construction do this assessment and cost estimate for them as part of their bid so I know that Story Construction is familiar with what will need done and is capable of performing the work.
